Under the guidelines for Colorado’s Division IME program, effective January 1, 2019, there are specific rules regarding compiling the medical records and coordination between parties to assure that the examining physician receives the most complete set of records possible in a single packet of records. These rules require strict adherence to guidelines regarding content and timing for preparation of the medical records.
Medical Utilization Review - The rules for preparing records for Utilization Review are very specific about the format and what may or may not be included in the packet of records. Our staff is specifically trained regarding these requirements, and we have been providing this service since 1999.
A review of the file summarizing the injuries and treatment must be performed by a licensed medical professional in accordance with Rule 10. The medical records are sorted into the appropriate sections and subsections as mandated by the guidelines and a Table of Contents is prepared properly identifying each record. Finally, 7 identical copies of the medical records packet are prepared, tabbed and bound in accordance with the rule. A Provider List is prepared for the required Information Packet and attached to all the Orders and Admissions for the claim.

Records are thoroughly reviewed and a case summary is prepared describing any injury events, medical conditions, treatment, responses to treatment and progression of the patient’s case. This service can be of significant benefit when you are trying to make sense of a complex case, or preparing to send a large case for independent evaluation or litigation referral.
Individual records, events, treatments etc. are summarized and dictated into strict chronological order. Medical treatment notes are put into S.O.A.P. note format if possible and highlights of relevant events or other injuries are briefly summarized into the chronology. Non-medical records may also be incorporated according to your personal preference.
